write-zeros has a bio, but does not have any data buffers associated

nvme: add 48-bit DMA address quirk for Amazon NVMe controllers
Some Amazon NVMe controllers do not follow the NVMe specification
and are limited to 48-bit DMA addresses. Add a quirk to force
bounce buffering if needed and limit the IOVA allocation for these
devices.
This affects all current Amazon NVMe controllers that expose EBS
volumes (0x0061, 0x0065, 0x8061) and local instance storage
(0xcd00, 0xcd01, 0xcd02).

nvme-hwmon: rework to avoid devm allocation
The original design to use device-managed resource allocation
doesn't really work as the NVMe controller has a vastly different
lifetime than the hwmon sysfs attributes, causing warning about
duplicate sysfs entries upon reconnection.
This patch reworks the hwmon allocation to avoid device-managed
resource allocation, and uses the NVMe controller as parent for
the sysfs attributes.

nvmet: remove else at the end of the function
The function nvmet_parse_io_cmd() returns value from
nvmet_file_parse_io_cmd() or nvmet_bdev_parse_io_cmd() based on which
backend is set for the request. Remove the else and just return the
value from nvmet_bdev_parse_io_cmd().

nvmet: make nvmet_find_namespace() req based
The six callers of nvmet_find_namespace() duplicate the error log page
update and status setting code for each call on failure.
All callers are nvmet requests based functions, so we can pass req
to the nvmet_find_namesapce() & derive ctrl from req, that'll allow us
to update the error log page in nvmet_find_namespace(). Now that we
pass the request we can also get rid of the local variable in
nvmet_find_namespace() and use the req->ns and return the error code.
Replace the ctrl parameter with nvmet_req for nvmet_find_namespace(),
centralize the error log page update for non allocated namesapces, and
return uniform error for non-allocated namespace.
The nvmet_find_namespace() takes nsid parameter which is from NVMe
commands structures such as get_log_page, identify, rw and common. All
these commands have same offset for the nsid field.
Derive nsid from req->cmd->common.nsid) & remove the extra parameter
from the nvmet_find_namespace().
Lastly now we associate the ns to the req parameter that we pass to the
nvmet_find_namespace(), rename nvmet_find_namespace() to
nvmet_req_find_ns().

nvmet: return uniform error for invalid ns
For nvmet_find_namespace() error case we have inconsistent error code
mapping in the function nvmet_get_smart_log_nsid() and
nvmet_set_feat_write_protect().
There is no point in retrying for the invalid namesapce from the host
side. Set the error code to the NVME_SC_INVALID_NS | NVME_SC_DNR which
matches what we have in nvmet_execute_identify_desclist().

nvmet-tcp: fix potential race of tcp socket closing accept_work
When we accept a TCP connection and allocate an nvmet-tcp queue we should
make sure not to fully establish it or reference it as the connection may
be already closing, which triggers queue release work, which does not
fence against queue establishment.
In order to address such a race, we make sure to check the sk_state and
contain the queue reference to be done underneath the sk_callback_lock
such that the queue release work correctly fences against it.

nvme-rdma: handle nvme_rdma_post_send failures better
nvme_rdma_post_send failing is a path related error and should bounce
to another path when using nvme-multipath. Call nvme_host_path_error
when nvme_rdma_post_send returns -EIO to ensure nvme_complete_rq gets
invoked to fail over to another path if there is one.

nvme-fabrics: avoid double completions in nvmf_fail_nonready_command
When reconnecting, the request may be completed with
NVME_SC_HOST_PATH_ERROR in nvmf_fail_nonready_command, which currently
set the state of the request to MQ_RQ_IN_FLIGHT before calling
nvme_complete_rq. When this happens for a request that is freed by
the caller, such as nvme_submit_user_cmd, in the worst case the request
could be completed again in tear down process.
Instead of calling blk_mq_start_request from nvmf_fail_nonready_command,
just use the new nvme_host_path_error helper to complete the command
without starting it.

nvme: introduce a nvme_host_path_error helper
When using nvme native multipathing, if a path related error occurs
during ->queue_rq, the request needs to be completed with
NVME_SC_HOST_PATH_ERROR so that the request can be failed over.
Introduce a helper to complete the command from ->queue_rq in a wait
that invokes nvme_complete_rq.

bcache: Move journal work to new flush wq
This is potentially long running and not latency sensitive, let's get
it out of the way of other latency sensitive events.
As observed in the previous commit, the `system_wq` comes easily
congested by bcache, and this fixes a few more stalls I was observing
every once in a while.
Let's not make this `WQ_MEM_RECLAIM` as it showed to reduce performance
of boot and file system operations in my tests. Also, without
`WQ_MEM_RECLAIM`, I no longer see desktop stalls. This matches the
previous behavior as `system_wq` also does no memory reclaim:

bcache: Give btree_io_wq correct semantics again
Before killing `btree_io_wq`, the queue was allocated using
`create_singlethread_workqueue()` which has `WQ_MEM_RECLAIM`. After
killing it, it no longer had this property but `system_wq` is not
single threaded.
Let's combine both worlds and make it multi threaded but able to
reclaim memory.

Revert "bcache: Kill btree_io_wq"
This reverts commit
56b30770b27d54d68ad51eccc6d888282b568cee.
With the btree using the `system_wq`, I seem to see a lot more desktop
latency than I should.
After some more investigation, it looks like the original assumption
of
56b3077 no longer is true, and bcache has a very high potential of
congesting the `system_wq`. In turn, this introduces laggy desktop
performance, IO stalls (at least with btrfs), and input events may be
delayed.
So let's revert this. It's important to note that the semantics of
using `system_wq` previously mean that `btree_io_wq` should be created
before and destroyed after other bcache wqs to keep the same
assumptions.

bcache: consider the fragmentation when update the writeback rate
Current way to calculate the writeback rate only considered the
dirty sectors, this usually works fine when the fragmentation
is not high, but it will give us unreasonable small rate when
we are under a situation that very few dirty sectors consumed
a lot dirty buckets. In some case, the dirty bucekts can reached
to CUTOFF_WRITEBACK_SYNC while the dirty data(sectors) not even
reached the writeback_percent, the writeback rate will still
be the minimum value (4k), thus it will cause all the writes to be
stucked in a non-writeback mode because of the slow writeback.
We accelerate the rate in 3 stages with different aggressiveness,
the first stage starts when dirty buckets percent reach above
BCH_WRITEBACK_FRAGMENT_THRESHOLD_LOW (50), the second is
BCH_WRITEBACK_FRAGMENT_THRESHOLD_MID (57), the third is
BCH_WRITEBACK_FRAGMENT_THRESHOLD_HIGH (64). By default
the first stage tries to writeback the amount of dirty data
in one bucket (on average) in (1 / (dirty_buckets_percent - 50)) second,
the second stage tries to writeback the amount of dirty data in one bucket
in (1 / (dirty_buckets_percent - 57)) * 100 millisecond, the third
stage tries to writeback the amount of dirty data in one bucket in
(1 / (dirty_buckets_percent - 64)) millisecond.
the initial rate at each stage can be controlled by 3 configurable
parameters writeback_rate_fp_term_{low|mid|high}, they are by default
1, 10, 1000, the hint of IO throughput that these values are trying
to achieve is described by above paragraph, the reason that
I choose those value as default is based on the testing and the
production data, below is some details:
A. When it comes to the low stage, there is still a bit far from the 70
threshold, so we only want to give it a little bit push by setting the
term to 1, it means the initial rate will be 170 if the fragment is 6,
it is calculated by bucket_size/fragment, this rate is very small,
but still much reasonable than the minimum 8.
For a production bcache with unheavy workload, if the cache device
is bigger than 1 TB, it may take hours to consume 1% buckets,
so it is very possible to reclaim enough dirty buckets in this stage,
thus to avoid entering the next stage.
B. If the dirty buckets ratio didn't turn around during the first stage,
it comes to the mid stage, then it is necessary for mid stage
to be more aggressive than low stage, so i choose the initial rate
to be 10 times more than low stage, that means 1700 as the initial
rate if the fragment is 6. This is some normal rate
we usually see for a normal workload when writeback happens
because of writeback_percent.
C. If the dirty buckets ratio didn't turn around during the low and mid
stages, it comes to the third stage, and it is the last chance that
we can turn around to avoid the horrible cutoff writeback sync issue,
then we choose 100 times more aggressive than the mid stage, that
means 170000 as the initial rate if the fragment is 6. This is also
inferred from a production bcache, I've got one week's writeback rate
data from a production bcache which has quite heavy workloads,
again, the writeback is triggered by the writeback percent,
the highest rate area is around 100000 to 240000, so I believe this
kind aggressiveness at this stage is reasonable for production.
And it should be mostly enough because the hint is trying to reclaim
1000 bucket per second, and from that heavy production env,
it is consuming 50 bucket per second on average in one week's data.
Option writeback_consider_fragment is to control whether we want
this feature to be on or off, it's on by default.
Lastly, below is the performance data for all the testing result,
including the data from production env:

sd_zbc: clear zone resources for non-zoned case
For host-aware ZBC disk, setting the device zoned model to BLK_ZONED_HA
using blk_queue_set_zoned() in sd_read_block_characteristics() may
result in the block device effective zoned model to be "none"
(BLK_ZONED_NONE) if partitions are present on the device. In this case,
sd_zbc_read_zones() should not setup the zone related queue limits for
the disk so that the device limits and configuration is consistent with
a regular disk and resources not uselessly allocated (e.g. the zone
write pointer tracking array for zone append emulation).
Furthermore, if the disk zoned model changes at run time due to the
creation of a partition by the user, the zone related resources can be
released.
Fix both problems by introducing the function sd_zbc_clear_zone_info()
to reset the scsi disk zone information and free resources and by
returning early in sd_zbc_read_zones() for a block device that has a
zoned model equal to BLK_ZONED_NONE.

cleanup all limits and resources related to zoned block devices. This
new function is called from blk_queue_set_zoned() when a disk zoned
model is set to BLK_ZONED_NONE. This particular case can happens when a
partition is created on a host-aware scsi disk.

zonefs: use zone write granularity as block size
Zoned block devices have different granularity constraints for write
operations into sequential zones. E.g. ZBC and ZAC devices require that
writes be aligned to the device physical block size while NVMe ZNS
devices allow logical block size aligned write operations. To correctly
handle such difference, use the device zone write granularity limit to
set the block size of a zonefs volume, thus allowing the smallest
possible write unit for all zoned device types.

block: introduce zone_write_granularity limit
Per ZBC and ZAC specifications, host-managed SMR hard-disks mandate that
all writes into sequential write required zones be aligned to the device
physical block size. However, NVMe ZNS does not have this constraint and
allows write operations into sequential zones to be aligned to the
device logical block size. This inconsistency does not help with
software portability across device types.
To solve this, introduce the zone_write_granularity queue limit to
indicate the alignment constraint, in bytes, of write operations into
zones of a zoned block device. This new limit is exported as a
read-only sysfs queue attribute and the helper
blk_queue_zone_write_granularity() introduced for drivers to set this
limit.
The function blk_queue_set_zoned() is modified to set this new limit to
the device logical block size by default. NVMe ZNS devices as well as
zoned nullb devices use this default value as is. The scsi disk driver
is modified to execute the blk_queue_zone_write_granularity() helper to
set the zone write granularity of host-managed SMR disks to the disk
physical block size.
The accessor functions queue_zone_write_granularity() and
bdev_zone_write_granularity() are also introduced.

Use blk_queue_set_zoned() to set a nullb device zone model instead of
directly assigning the device queue zoned limit. This initialization of
the devicve zoned model as well as the setup of the queue flag
QUEUE_FLAG_ZONE_RESETALL and of the device queue elevator feature are
moved from null_init_zoned_dev() to null_register_zoned_dev() so that
the initialization of the queue limits is done when the gendisk of the
nullb device is available.

nvme: cleanup zone information initialization
For a zoned namespace, in nvme_update_ns_info(), call
nvme_update_zone_info() after executing nvme_update_disk_info() so that
the namespace queue logical and physical block size limits are set.
This allows setting the namespace queue max_zone_append_sectors limit
in nvme_update_zone_info() instead of nvme_revalidate_zones(),
simplifying this function. Also use blk_queue_set_zoned() to set the
namespace zoned model.

x86/efi: Remove EFI PGD build time checks
With CONFIG_X86_5LEVEL, CONFIG_UBSAN and CONFIG_UBSAN_UNSIGNED_OVERFLOW
enabled, clang fails the build with
x86_64-linux-ld: arch/x86/platform/efi/efi_64.o: in function `efi_sync_low_kernel_mappings':
efi_64.c:(.text+0x22c): undefined reference to `__compiletime_assert_354'
which happens due to -fsanitize=unsigned-integer-overflow being enabled:
-fsanitize=unsigned-integer-overflow: Unsigned integer overflow, where
the result of an unsigned integer computation cannot be represented
in its type. Unlike signed integer overflow, this is not undefined
behavior, but it is often unintentional. This sanitizer does not check
for lossy implicit conversions performed before such a computation
(see -fsanitize=implicit-conversion).
and that fires when the (intentional) EFI_VA_START/END defines overflow
an unsigned long, leading to the assertion expressions not getting
optimized away (on GCC they do)...
However, those checks are superfluous: the runtime services mapping
code already makes sure the ranges don't overshoot EFI_VA_END as the
EFI mapping range is hardcoded. On each runtime services call, it is
switched to the EFI-specific PGD and even if mappings manage to escape
that last PGD, this won't remain unnoticed for long.
So rip them out.

entry: Use different define for selector variable in SUD
Michael Kerrisk suggested that, from an API perspective, it is a bad
idea to share the PR_SYS_DISPATCH_ defines between the prctl operation
and the selector variable.
Therefore, define two new constants to be used by SUD's selector variable
and update the corresponding documentation and test cases.
While this changes the API syscall user dispatch has never been part of a
Linux release, it will show up for the first time in 5.11.

entry: Ensure trap after single-step on system call return
Commit
299155244770 ("entry: Drop usage of TIF flags in the generic syscall
code") introduced a bug on architectures using the generic syscall entry
code, in which processes stopped by PTRACE_SYSCALL do not trap on syscall
return after receiving a TIF_SINGLESTEP.
The reason is that the meaning of TIF_SINGLESTEP flag is overloaded to
cause the trap after a system call is executed, but since the above commit,
the syscall call handler only checks for the SYSCALL_WORK flags on the exit
work.
Split the meaning of TIF_SINGLESTEP such that it only means single-step
mode, and create a new type of SYSCALL_WORK to request a trap immediately
after a syscall in single-step mode. In the current implementation, the
SYSCALL_WORK flag shadows the TIF_SINGLESTEP flag for simplicity.
Update x86 to flip this bit when a tracer enables single stepping.

This reverts commit

This change is broken and not solving any problem it claims to solve.
Robin reported that cpumask_local_spread() now returns any cpu out of
cpu_possible_mask in case that NOHZ_FULL is disabled (runtime or compile
time). It can also return any offline or not-present CPU in the
housekeeping mask. Before that it was returning a CPU out of
online_cpu_mask.
While the function is racy against CPU hotplug if the caller does not
protect against it, the actual use cases are not caring much about it as
they use it mostly as hint for:
- the user space affinity hint which is unused by the kernel
- memory node selection which is just suboptimal
- network queue affinity which might fail but is handled gracefully
But the occasional fail vs. hotplug is very different from returning
anything from possible_cpu_mask which can have a large amount of offline
CPUs obviously.
The changelog of the commit claims:
"The current implementation of cpumask_local_spread() does not respect
the isolated CPUs, i.e., even if a CPU has been isolated for Real-Time
task, it will return it to the caller for pinning of its IRQ
threads. Having these unwanted IRQ threads on an isolated CPU adds up
to a latency overhead."
The only correct part of this changelog is:
"The current implementation of cpumask_local_spread() does not respect
the isolated CPUs."
Everything else is just disjunct from reality.

tracing: Do not count ftrace events in top level enable output
The file /sys/kernel/tracing/events/enable is used to enable all events by
echoing in "1", or disabling all events when echoing in "0". To know if all
events are enabled, disabled, or some are enabled but not all of them,
cating the file should show either "1" (all enabled), "0" (all disabled), or
"X" (some enabled but not all of them). This works the same as the "enable"
files in the individule system directories (like tracing/events/sched/enable).
But when all events are enabled, the top level "enable" file shows "X". The
reason is that its checking the "ftrace" events, which are special events
that only exist for their format files. These include the format for the
function tracer events, that are enabled when the function tracer is
enabled, but not by the "enable" file. The check includes these events,
which will always be disabled, and even though all true events are enabled,
the top level "enable" file will show "X" instead of "1".
To fix this, have the check test the event's flags to see if it has the
"IGNORE_ENABLE" flag set, and if so, not test it.

genirq: Prevent [devm_]irq_alloc_desc from returning irq 0
Since commit
a85a6c86c25b ("driver core: platform: Clarify that IRQ 0
is invalid"), having a linux-irq with number 0 will trigger a WARN()
when calling platform_get_irq*() to retrieve that linux-irq.
Since [devm_]irq_alloc_desc allocs a single irq and since irq 0 is not used
on some systems, it can return 0, triggering that WARN(). This happens
e.g. on Intel Bay Trail and Cherry Trail devices using the LPE audio engine
for HDMI audio:
0 is an invalid IRQ number

cifs: report error instead of invalid when revalidating a dentry fails
Assuming
- //HOST/a is mounted on /mnt
- //HOST/b is mounted on /mnt/b
On a slow connection, running 'df' and killing it while it's
processing /mnt/b can make cifs_get_inode_info() returns -ERESTARTSYS.
This triggers the following chain of events:
=> the dentry revalidation fail
=> dentry is put and released
=> superblock associated with the dentry is put
=> /mnt/b is unmounted
This patch makes cifs_d_revalidate() return the error instead of 0
(invalid) when cifs_revalidate_dentry() fails, except for ENOENT (file
deleted) and ESTALE (file recreated).

x86/debug: Prevent data breakpoints on cpu_dr7
local_db_save() is called at the start of exc_debug_kernel(), reads DR7 and
disables breakpoints to prevent recursion.
When running in a guest (X86_FEATURE_HYPERVISOR), local_db_save() reads the
per-cpu variable cpu_dr7 to check whether a breakpoint is active or not
before it accesses DR7.
A data breakpoint on cpu_dr7 therefore results in infinite #DB recursion.
Disallow data breakpoints on cpu_dr7 to prevent that.

x86/debug: Prevent data breakpoints on __per_cpu_offset
When FSGSBASE is enabled, paranoid_entry() fetches the per-CPU GSBASE value
via __per_cpu_offset or pcpu_unit_offsets.
When a data breakpoint is set on __per_cpu_offset[cpu] (read-write
operation), the specific CPU will be stuck in an infinite #DB loop.
RCU will try to send an NMI to the specific CPU, but it is not working
either since NMI also relies on paranoid_entry(). Which means it's
undebuggable.

kasan: make addr_has_metadata() return true for valid addresses
Currently, addr_has_metadata() returns true for every address. An
invalid address (e.g. NULL) passed to the function when, KASAN_HW_TAGS
is enabled, leads to a kernel panic.
Make addr_has_metadata() return true for valid addresses only.
Note: KASAN_HW_TAGS support for vmalloc will be added with a future
patch.

kasan: add explicit preconditions to kasan_report()
Patch series "kasan: Fix metadata detection for KASAN_HW_TAGS", v5.
With the introduction of KASAN_HW_TAGS, kasan_report() currently assumes
that every location in memory has valid metadata associated. This is
due to the fact that addr_has_metadata() returns always true.
As a consequence of this, an invalid address (e.g. NULL pointer
address) passed to kasan_report() when KASAN_HW_TAGS is enabled, leads
to a kernel panic.

Before that commit, there was a try_charge() and commit_charge() in
__add_to_page_cache_locked(). These two separated charge functions were
replaced by a single mem_cgroup_charge(). However, it forgot to add a
matching mem_cgroup_uncharge() when the xarray insertion failed with the
page released back to the pool.
Fix this by adding a mem_cgroup_uncharge() call when insertion error
happens.

At the same time, the kernel image is protected with memblock_reserve(),
so we can just start searching at PAGE_SIZE. In this case the bottom-up
allocation has the same chances to success as a top-down allocation, so
there is no reason to fallback in the case of a failure. All together it
simplifies the logic.

mm: thp: fix MADV_REMOVE deadlock on shmem THP
Sergey reported deadlock between kswapd correctly doing its usual
lock_page(page) followed by down_read(page->mapping->i_mmap_rwsem), and
madvise(MADV_REMOVE) on an madvise(MADV_HUGEPAGE) area doing
down_write(page->mapping->i_mmap_rwsem) followed by lock_page(page).
This happened when shmem_fallocate(punch hole)'s unmap_mapping_range()
reaches zap_pmd_range()'s call to __split_huge_pmd(). The same deadlock
could occur when partially truncating a mapped huge tmpfs file, or using
fallocate(FALLOC_FL_PUNCH_HOLE) on it.
__split_huge_pmd()'s page lock was added in 5.8, to make sure that any
concurrent use of reuse_swap_page() (holding page lock) could not catch
the anon THP's mapcounts and swapcounts while they were being split.
Fortunately, reuse_swap_page() is never applied to a shmem or file THP
(not even by khugepaged, which checks PageSwapCache before calling), and
anonymous THPs are never created in shmem or file areas: so that
__split_huge_pmd()'s page lock can only be necessary for anonymous THPs,
on which there is no risk of deadlock with i_mmap_rwsem.

init/gcov: allow CONFIG_CONSTRUCTORS on UML to fix module gcov
On ARCH=um, loading a module doesn't result in its constructors getting
called, which breaks module gcov since the debugfs files are never
registered. On the other hand, in-kernel constructors have already been
called by the dynamic linker, so we can't call them again.
Get out of this conundrum by allowing CONFIG_CONSTRUCTORS to be
selected, but avoiding the in-kernel constructor calls.
Also remove the "if !UML" from GCOV selecting CONSTRUCTORS now, since we
really do want CONSTRUCTORS, just not kernel binary ones.

mm/vmalloc: separate put pages and flush VM flags
When VM_MAP_PUT_PAGES was added, it was defined with the same value as
VM_FLUSH_RESET_PERMS. This doesn't seem like it will cause any big
functional problems other than some excess flushing for VM_MAP_PUT_PAGES
allocations.
Redefine VM_MAP_PUT_PAGES to have its own value. Also, rearrange things
so flags are less likely to be missed in the future.

mm, compaction: move high_pfn to the for loop scope
In fast_isolate_freepages, high_pfn will be used if a prefered one (ie
PFN >= low_fn) not found.
But the high_pfn is not reset before searching an free area, so when it
was used as freepage, it may from another free area searched before. As
a result move_freelist_head(freelist, freepage) will have unexpected

The issue was reported on an smart phone product with 6GB ram and 3GB
zram as swap device.
This patch fixes the issue by reset high_pfn before searching each free
area, which ensure freepage and freelist match when call

mm: migrate: do not migrate HugeTLB page whose refcount is one
All pages isolated for the migration have an elevated reference count and
therefore seeing a reference count equal to 1 means that the last user of
the page has dropped the reference and the page has became unused and
there doesn't make much sense to migrate it anymore.
This has been done for regular pages and this patch does the same for
hugetlb pages. Although the likelihood of the race is rather small for
hugetlb pages it makes sense the two code paths in sync.

mm: hugetlb: remove VM_BUG_ON_PAGE from page_huge_active
The page_huge_active() can be called from scan_movable_pages() which do
not hold a reference count to the HugeTLB page. So when we call
page_huge_active() from scan_movable_pages(), the HugeTLB page can be
freed parallel. Then we will trigger a BUG_ON which is in the
page_huge_active() when CONFIG_DEBUG_VM is enabled. Just remove the
VM_BUG_ON_PAGE.

and dissolve_free_huge_page().

mm: hugetlbfs: fix cannot migrate the fallocated HugeTLB page
If a new hugetlb page is allocated during fallocate it will not be
marked as active (set_page_huge_active) which will result in a later
isolate_huge_page failure when the page migration code would like to
move that page. Such a failure would be unexpected and wrong.
Only export set_page_huge_active, just leave clear_page_huge_active as
static. Because there are no external users.
