ARM: SMP: ensure frame pointer is reinitialized for soft-CPU hotplug

When we soft-CPU hotplug a CPU, we reset the stack pointer and
jump back to start_secondary().  This allows us to restart as if
the CPU was actually reset.

However, we weren't resetting the frame pointer, which could cause
problems with backtracing.  Reset the frame pointer to zero (which
means no parent frame) just like the early assembly code also does.

ARM: localtimer: clean up local timer on hot unplug

When a CPU is hot unplugged, the generic tick code cleans up the
clock event device, but fails to call down to the device's set_mode
function to actually shut the device down.

To work around this, we've historically had a local_timer_stop()
callback out of the hotplug code.  However, this adds needless
complexity when we have the clock event device itself available.

Explicitly call the clock event device's set_mode function with
CLOCK_EVT_MODE_UNUSED, so that the hardware can be cleanly shutdown
without any special external callbacks.  When/if the generic code
is fixed, percpu_timer_stop() can be killed off.

ARM: CPU hotplug: fix reporting of spurious wakeups

The original scheme for reporting spurious wakeups was broken - it
tried to use printk() from a context which wasn't coherent with the
other CPUs, which risks corrupting the printk() data.

Fix this by noting the number spurious wakeups, and only report them
when we are properly woken - when we will be coherent with the rest
of the system.

ARM: smp: improve CPU bringup failure diagnostics

We used to print a bland error message which gave no clue as to the
failure when we failed to bring up a secondary CPU.  Resolve this by
separating the two failure cases.

If boot_secondary() fails, we print a message indicating the returned
error code from boot_secondary():
"CPU%u: failed to boot: %d\n", cpu, ret.

However, if boot_secondary() succeeded, but the CPU did not appear to
mark itself online within the timeout, indicate that it failed to come
online:
"CPU%u: failed to come online\n", cpu

ARM: Fix subtle race in CPU pen_release hotplug code

There is a subtle race in the CPU hotplug code, where a CPU which has
been offlined can online itself before being requested, which results
in things going astray on the next online/offline cycle.

What happens in the normal online/offline/online cycle is:

CPU0 CPU3
requests boot of CPU3
pen_release = 3
flush cache line
checks pen_release, reads 3
starts boot
pen_release = -1
... requests CPU3 offline ...
... dies ...
checks pen_release, reads -1
requests boot of CPU3
pen_release = 3
flush cache line
checks pen_release, reads 3
starts boot
pen_release = -1

However, as the write of -1 of pen_release is not fully flushed back to
memory, and the checking of pen_release is done with caches disabled,
this allows CPU3 the opportunity to read the old value of pen_release:

CPU0 CPU3
requests boot of CPU3
pen_release = 3
flush cache line
checks pen_release, reads 3
starts boot
pen_release = -1
... requests CPU3 offline ...
... dies ...
checks pen_release, reads 3
starts boot
pen_release = -1
requests boot of CPU3

flush cache line

Fix this by grouping the write of pen_release along with its cache line
flushing code to ensure that any update to pen_release is always pushed
out to physical memory.

ARM: 6516/1: Allow SMP_ON_UP to work with Thumb-2 kernels.

  * __fixup_smp_on_up has been modified with support for the
    THUMB2_KERNEL case.  For THUMB2_KERNEL only, fixups are split
    into halfwords in case of misalignment, since we can't rely on
    unaligned accesses working before turning the MMU on.

    No attempt is made to optimise the aligned case, since the
    number of fixups is typically small, and it seems best to keep
    the code as simple as possible.

  * Add a rotate in the fixup_smp code in order to support
    CPU_BIG_ENDIAN, as suggested by Nicolas Pitre.

  * Add an assembly-time sanity-check to ALT_UP() to ensure that
    the content really is the right size (4 bytes).

    (No check is done for ALT_SMP().  Possibly, this could be fixed
    by splitting the two uses ot ALT_SMP() (ALT_SMP...SMP_UP versus
    ALT_SMP...SMP_UP_B) into two macros.  In the first case,
    ALT_SMP needs to expand to >= 4 bytes, not == 4.)

  * smp_mpidr.h (which implements ALT_SMP()/ALT_UP() manually due
    to macro limitations) has not been modified: the affected
    instruction (mov) has no 16-bit encoding, so the correct
    instruction size is satisfied in this case.

  * A "mode" parameter has been added to smp_dmb:

    smp_dmb arm @ assumes 4-byte instructions (for ARM code, e.g. kuser)
    smp_dmb     @ uses W() to ensure 4-byte instructions for ALT_SMP()

    This avoids assembly failures due to use of W() inside smp_dmb,
    when assembling pure-ARM code in the vectors page.

    There might be a better way to achieve this.

  * Kconfig: make SMP_ON_UP depend on
    (!THUMB2_KERNEL || !BIG_ENDIAN) i.e., THUMB2_KERNEL is now
    supported, but only if !BIG_ENDIAN (The fixup code for Thumb-2
    currently assumes little-endian order.)

Tested using a single generic realview kernel on:

exec: copy-and-paste the fixes into compat_do_execve() paths

Note: this patch targets 2.6.37 and tries to be as simple as possible.
That is why it adds more copy-and-paste horror into fs/compat.c and
uglifies fs/exec.c, this will be cleanuped later.

compat_copy_strings() plays with bprm->vma/mm directly and thus has
two problems: it lacks the RLIMIT_STACK check and argv/envp memory
is not visible to oom killer.

Export acct_arg_size() and get_arg_page(), change compat_copy_strings()
to use get_arg_page(), change compat_do_execve() to do acct_arg_size(0)
as do_execve() does.

Add the fatal_signal_pending/cond_resched checks into compat_count() and
compat_copy_strings(), this matches the code in fs/exec.c and certainly
makes sense.

exec: make argv/envp memory visible to oom-killer

Brad Spengler published a local memory-allocation DoS that
evades the OOM-killer (though not the virtual memory RLIMIT):

execve()->copy_strings() can allocate a lot of memory, but
this is not visible to oom-killer, nobody can see the nascent
bprm->mm and take it into account.

With this patch get_arg_page() increments current's MM_ANONPAGES
counter every time we allocate the new page for argv/envp. When
do_execve() succeds or fails, we change this counter back.

Technically this is not 100% correct, we can't know if the new
page is swapped out and turn MM_ANONPAGES into MM_SWAPENTS, but
I don't think this really matters and everything becomes correct
once exec changes ->mm or fails.

Revert "debug_locks: set oops_in_progress if we will log messages."

This reverts commit e0fdace10e75dac67d906213b780ff1b1a4cc360.

On-list discussion seems to suggest that the robustness fixes for printk
make this unnecessary and DaveM has also agreed in person at Kernel Summit
and on list.

The main problem with this code is once we hit a lockdep splat we always
keep oops_in_progress set, the console layer uses oops_in_progress with KMS
to decide when it should be showing the oops and not showing X, so it causes
problems around suspend/resume time when a userspace resume can cause a console
switch away from X, only if oops_in_progress is set (which is what we want
if an oops actually is in progress, but not because we had a lockdep splat
2 days prior).

tpm: Autodetect itpm devices

Some Lenovos have TPMs that require a quirk to function correctly. This can
be autodetected by checking whether the device has a _HID of INTC0102. This
is an invalid PNPid, and as such is discarded by the pnp layer - however
it's still present in the ACPI code, so we can pull it out that way. This
means that the quirk won't be automatically applied on non-ACPI systems,
but without ACPI we don't have any way to identify the chip anyway so I
don't think that's a great concern.

af_unix: limit recursion level

Its easy to eat all kernel memory and trigger NMI watchdog, using an
exploit program that queues unix sockets on top of others.

This mechanism is used in applications, one choice we have is to have a
recursion limit.

Other limits might be needed as well (if we queue other types of files),
since the passfd mechanism is currently limited by socket receive queue
sizes only.

Add a recursion_level to unix socket, allowing up to 4 levels.

Each time we send an unix socket through sendfd mechanism, we copy its
recursion level (plus one) to receiver. This recursion level is cleared
when socket receive queue is emptied.

ucc_geth: fix ucc halt problem in half duplex mode

In commit 58933c64(ucc_geth: Fix the wrong the Rx/Tx FIFO size),
the UCC_GETH_UTFTT_INIT is set to 512 based on the recommendation
of the QE Reference Manual.  But that will sometimes cause tx halt
while working in half duplex mode.

According to errata draft QE_GENERAL-A003(High Tx Virtual FIFO
threshold size can cause UCC to halt), setting UTFTT less than
[(UTFS x (M - 8)/M) - 128] will prevent this from happening
(M is the minimum buffer size).

The patch changes UTFTT back to 256.

inet: Fix __inet_inherit_port() to correctly increment bsockets and num_owners

inet sockets corresponding to passive connections are added to the bind hash
using ___inet_inherit_port(). These sockets are later removed from the bind
hash using __inet_put_port(). These two functions are not exactly symmetrical.
__inet_put_port() decrements hashinfo->bsockets and tb->num_owners, whereas
___inet_inherit_port() does not increment them. This results in both of these
going to -ve values.

This patch fixes this by calling inet_bind_hash() from ___inet_inherit_port(),
which does the right thing.

'bsockets' and 'num_owners' were introduced by commit a9d8f9110d7e953c
(inet: Allowing more than 64k connections and heavily optimize bind(0))

Btrfs: deal with DIO bios that span more than one ordered extent

The new DIO bio splitting code has problems when the bio
spans more than one ordered extent.  This will happen as the
generic DIO code merges our get_blocks calls together into
a bigger single bio.

This fixes things by walking forward in the ordered extent
code finding all the overlapping ordered extents and completing them
all at once.

Export 'get_pipe_info()' to other users

And in particular, use it in 'pipe_fcntl()'.

The other pipe functions do not need to use the 'careful' version, since
they are only ever called for things that are already known to be pipes.

The normal read/write/ioctl functions are called through the file
operations structures, so if a file isn't a pipe, they'd never get
called.  But pipe_fcntl() is special, and called directly from the
generic fcntl code, and needs to use the same careful function that the
splice code is using.

Rename 'pipe_info()' to 'get_pipe_info()'

.. and change it to take the 'file' pointer instead of an inode, since
that's what all users want anyway.

The renaming is preparatory to exporting it to other users.  The old
'pipe_info()' name was too generic and is already used elsewhere, so
before making the function public we need to use a more specific name.

NET: wan/x25_asy, move lapb_unregister to x25_asy_close_tty

We register lapb when tty is created, but unregister it only when the
device is UP. So move the lapb_unregister to x25_asy_close_tty after
the device is down.

The old behaviour causes ldisc switching to fail each second attempt,
because we noted for us that the device is unused, so we use it the
second time, but labp layer still have it registered, so it fails
obviously.

Allocating unit from ird might return several error codes
not only -EAGAIN, so it should not be changed and returned
precisely. Same time unit release procedure should be invoked

tcp_win_from_space() does the following:

      if (sysctl_tcp_adv_win_scale <= 0)
              return space >> (-sysctl_tcp_adv_win_scale);
      else
              return space - (space >> sysctl_tcp_adv_win_scale);

"space" is int.

As per C99 6.5.7 (3) shifting int for 32 or more bits is
undefined behaviour.

Indeed, if sysctl_tcp_adv_win_scale is exactly 32,
space >> 32 equals space and function returns 0.

Which means we busyloop in tcp_fixup_rcvbuf().

Restrict net.ipv4.tcp_adv_win_scale to [-31, 31].

Btrfs: setup blank root and fs_info for mount time

There is a problem with how we use sget, it searches through the list of supers
attached to the fs_type looking for a super with the same fs_devices as what
we're trying to mount.  This depends on sb->s_fs_info being filled, but we don't
fill that in until we get to btrfs_fill_super, so we could hit supers on the
fs_type super list that have a null s_fs_info.  In order to fix that we need to
go ahead and setup a blank root with a blank fs_info to hold fs_devices, that
way our test will work out right and then we can set s_fs_info in
btrfs_set_super, and then open_ctree will simply use our pre-allocated root and
fs_info when setting everything up.  Thanks,

Btrfs: fix fiemap

There are two big problems currently with FIEMAP

1) We return extents for holes.  This isn't supposed to happen, we just don't
return extents for holes and then userspace interprets the lack of an extent as
a hole.

2) We sometimes don't set FIEMAP_EXTENT_LAST properly.  This is because we wait
to see a EXTENT_FLAG_VACANCY flag on the em, but this won't happen if say we ask
fiemap to map up to the last extent in a file, and there is nothing but holes up
to the i_size.  To fix this we need to lookup the last extent in this file and
save the logical offset, so if we happen to try and map that extent we can be
sure to set FIEMAP_EXTENT_LAST.

With this patch we now pass xfstest 225, which we never have before.

Btrfs - fix race between btrfs_get_sb() and umount

When mounting a btrfs file system btrfs_test_super() may attempt to
use sb->s_fs_info, the btrfs root, of a super block that is going away
and that has had the btrfs root set to NULL in its ->put_super(). But
if the super block is going away it cannot be an existing super block
so we can return false in this case.

Btrfs: make sure new inode size is ok in fallocate

We have been failing xfstest 228 forever, because we don't check to make sure
the new inode size is acceptable as far as RLIMIT is concerned.  Just check to
make sure it's ok to create a inode with this new size and error out if not.
With this patch we now pass 228.

Btrfs: fix typo in fallocate to make it honor actual size

There is a typo in __btrfs_prealloc_file_range() where we set the i_size to
actual_len/cur_offset, and then just set it to cur_offset again, and do the same
with btrfs_ordered_update_i_size().  This fixes it back to keeping i_size in a
local variable and then updating i_size properly.  Tested this with

xfs_io -F -f -c "falloc 0 1" -c "pwrite 0 1" foo

stat'ing foo gives us a size of 1 instead of 4096 like it was.  Thanks,
